Whole Person Care Community Services Worker III

The County of Ventura Health Care Agency is seeking to fill multiple positions for their California Whole Person Care Program. The Community Services Worker III will assist professional staff in providing transportation, referral, translation, and program related services to culturally, economically, physically, or mentally impaired or disadvantaged individuals in the community and performs other related duties as required.

The California Whole Person Care (WPC) Program is designed to coordinate health, behavioral health, and social services in order to improve the health outcomes of eligible Medi-Cal beneficiaries. Through collaboration and coordination among county agencies, health plans, providers and other entities, the WPC Pilots are designing and developing the infrastructure and processes to integrate and improve care for vulnerable populations, such as  those suffering from homelessness, mental illness, substance abuse, multiple chronic health conditions, and those who have been recently incarcerated.

The Community Services Worker III's will work with a multi-disciplinary team of professional and non-professional staff to provide intensive field-based care coordination and to implement an integrated care plan for high-utilizers of the health care system. 

This classification is eligible for overtime compensation.

EXAMPLES OF DUTIES:

• Manages a caseload of individuals with health, behavioral health, and social service needs; uses screening tools to inform, implement, and monitor plan of care for assigned patients; maintains health boundaries and professional ethics;
• Refers individuals and families to available community educational, medical, behavioral health, housing, public benefits, legal and employment resources; follows-up on referrals or appointments scheduled;
• Supports community outreach events serving at-risk populations including the homeless and mentally ill, effectively engaging clients and connecting them with needed resources;
• Drives County vehicles to pick up, transport and deliver clients to County buildings or Community Agency and/or deliver supplies;
• Coordinates efforts with internal and external multidisciplinary professionals working collaboratively to achieve positive outcomes for shared clients;
• Keeps records of clients served, payments received and clinic activities; orders supplies and prepares routine reports documenting encounters in the electronic health record to Agency standards;
• Provides translation and/or services, schedules appointments, obtains food and supplies for clients who are physically or mentally impaired;
• Answers inquiries and explains agency/program policies to individuals and groups within the community;
• May lead group presentations and workshops on specialized topics including substance abuse, home management skills, job skills or health and nutrition to help individuals and families develop a healthier or more self-sufficient lifestyle; 
• May supervise clients during approved visitations or activities; and
• Performs other related duties as required.

MINIMUM REQUIREMENTS:
Any combination of education and experience which has led to the acquisition of the required knowledge and abilities, which can be demonstrated by three (3) years' public social service experience or one (1) year of experience equivalent to that of a Community Services Worker II.

Substitution:

Fifteen (15) semester units of college course work in social sciences, ethnic studies,  communications, or a closely related field may be substituted for six (6) months of experience up to a total of one year.

If using the education substitution to qualify applicants must provide transcripts from an accredited institution. Submit by attaching to your application your transcripts/grade reports which shows your name, the number of units completed, and/or conferred date of a degree.  Unofficial transcripts are acceptable. If you need additional time to submit transcripts, contact the analyst conducting the recruitment for an extension. 

Coursework from a foreign institution require an academic credential evaluation to determine the U.S. equivalency.

NECESSARY SPECIAL REQUIREMENTS:

• A valid California driver license.
• May be required to work evenings, weekends, and holidays as needed.
• Experience with application assistance for health coverage and connecting individuals with social service such as SSI/SSDI, housing resources, and other community resources.
• Experience working with at-risk populations including persons who are homeless, mentally ill, or with complex medical or social service needs.

DESIRED:

• Bilingual skills (English/Spanish)
• Health Education experience
• Experience enrolling eligible members in public assistance programs like MediCal, CalFresh, and CalWORKS
• Knowledge of Covered California health plan options
